Alzheimer’s and dementia care

Dementia is a set of symptoms that may include memory loss and difficulties with thinking, problem-solving or language. These changes are often small to start with, but for someone with dementia they can become severe enough to affect daily life. You may experience changes in their mood or behaviour and also agitation.

Our care team is able to support people with Alzheimer’s and dementia which enable them to live at home safely. During our care planning we will also look at how we can support the family through tough and challenging times. Our team are able to deal with challenging behaviour’s that may be presented at times when dementia progression may occur, all our care is provided with, great compassion and professionalism while placing dignity and privacy at the forefront of our care and support

  • We are committed to helping you and your loved one during this difficult time, by offering a professional and empathetic end of life care service.
  • Our palliative team will make sure that your loved one is kept comfortable in their last days of life, which will lessen the stress and strain on you and the family who are still coming to terms during this difficult time.
  • This will enable your loved one to return home from hospital and remain at home, in familiar surroundings, with their loved ones by their side.
  • The team will work very closely with the community nursing teams to ensure that every need is met from Personal Care, Pressure Management, Incontinence Management, Nutrition and Medication Needs

The live in care service provides not only the support required but also friendship, companionship and peace of mind for families.
